What was announced
CMS announced in May 2026 that the Part D portion of the BALANCE Model has been delayed indefinitely. In its place, the GLP-1 Bridge — originally scheduled to run to the end of 2026 — was extended through 31 December 2027.
BALANCE, standing for better approaches to lifestyle and nutrition for comprehensive health, was intended to lower prices by having CMS negotiate directly with manufacturers. It was the durable programme the Bridge was designed to bridge to.
Why an indefinite delay is different from a postponement
A delay with a new date is a schedule change. A delay without one is an open question about whether the programme happens at all.
For a patient starting treatment under the Bridge, that distinction is the whole issue. The Bridge is time-limited by design. If BALANCE arrives, coverage continues in some form. If it does not, coverage ends on 31 December 2027 and the statutory Part D exclusion on weight-loss drugs reasserts itself.
What patients should plan around
Eighteen months of access at $50 a month, followed by an unknown. The honest planning assumption is that coverage may not continue, because no programme has been confirmed to continue it.
That is uncomfortable given what the withdrawal evidence shows. SURMOUNT-4 randomised participants to continue tirzepatide or switch to placebo after a 36-week lead-in: those continuing lost a further 5.5% of body weight, those withdrawn regained 14.0%.
So the question worth raising with a prescriber before starting is not only whether treatment is indicated, but what the plan is if the copay disappears at the end of 2027. Self-pay alternatives at that point would be brand Zepbound at $299 to $449 direct, approved orals from $149, or compounded preparations if that market still exists — which is itself under active FDA decision.
The wider policy picture
Two things are moving in opposite directions. Access to approved products has expanded sharply: brand prices fell through late 2025, TrumpRx launched in February 2026 advertising direct access at reduced prices, two oral products reached market, and Medicare opened a door in July.
Meanwhile the compounded market, which absorbed roughly 30% of US GLP-1 supply at its 2024 peak, is being closed off through shortage delistings, warning letters and the proposed bulks exclusion.
Read together, the direction is a market consolidating around approved products at lower prices, with temporary public subsidy at the Medicare end. Whether that subsidy becomes permanent is the open question BALANCE was meant to answer.
What we will be watching
Any CMS announcement of a BALANCE start date or a further Bridge extension. Any Congressional action on the statutory Part D exclusion, which is the underlying constraint. And the FDA's final determination on the bulks proposal, which decides what the self-pay fallback looks like if coverage lapses.
